Description
REQUIREMENTS:
· Applicant Eligibility Verification: Applicant and Service Member (or Guardian) are
required to read, complete and sign the statement of certification on the last page of the
application. Unsigned applications will not be accepted.
· SAT/ACT Scores: Applicant must submit a copy or copies of ACT/SAT scores. The
highest scores will be considered.
· TRANSCRIPTS: Submit a sealed copy of your most recent transcript including your GPA.
· ESSAY: Type your essay on a separate sheet of paper.
· REFERENCES: Submit 3 personal references in envelopes sealed by the writer.
References may be mailed by the person giving the reference to the committee BUT MUST
CLEARLY STATE THE NAME OF THE APPLICANT, AND MUST BE RECEIVED
BY THE DEADLINE. Information should include length of time known, character and
leadership qualities and any other characteristics that make the applicant stand out.
References must be signed and dated.
· IF SERVICE MEMBER IS DECEASED, include copy of DD Form 1300 or death
certificate.
· IF SERVICE MEMBER WAS WOUNDED, RETURNED TO ACTIVE DUTY, AND
THEN RETIRED, Include copy of DD Form 214.
· IF SERVICE MEMBER WAS WOUNDED, BUT IS NOT RETIRED, include a
statement from their commanding officer.
